Manufacturing Standards: ISO 5 Cleanrooms and cGMP

What cleanroom classification and manufacturing-quality regulation actually require for biologic and tissue products — and how the requirements differ between a full 351(a) biologic and a 361 HCT/P.

What "ISO Class 5" means

ISO 14644-1 is the international cleanroom classification standard, referenced directly in FDA's own aseptic-processing guidance. FDA's 2004 guidance Sterile Drug Products Produced by Aseptic Processing reproduces the ISO correlation table used industry-wide:

In plain terms: ISO Class 5 permits no more than 3,520 particles of 0.5 microns or larger per cubic meter of air — roughly equivalent to the old "Class 100" standard and to EU Grade A, the cleanest classification typically used in pharmaceutical/biologic manufacturing. FDA's guidance states that samples from an ISO 5 (Class 100) environment "should normally yield no microbiological contaminants."

cGMP: 21 CFR Parts 210/211, plus biologics-specific rules

21 CFR Part 210 and Part 211 set the baseline Current Good Manufacturing Practice requirements for any drug product. Directly relevant to cleanrooms, 21 CFR § 211.42(c)(10) requires that aseptic processing areas include HEPA-filtered air under positive pressure, temperature/humidity controls, environmental-condition monitoring, validated cleaning/disinfection, and equipment-maintenance systems.

For CBER-licensed biologics, this baseline cGMP is layered with biologics-specific requirements at 21 CFR Parts 600–680. 21 CFR § 600.11 imposes additional facility standards distinct from Part 211 — orderly, clean, vermin-free work areas; ventilation arranged to prevent cross-contamination between manufacturing areas; validated sterilization equipment; and specific controls for spore-forming organisms in multiproduct facilities.

361 HCT/Ps: Current Good Tissue Practice (cGTP), not full cGMP

21 CFR § 1271.150 sets out Current Good Tissue Practice — a narrower standard whose stated purpose is specifically "to prevent the introduction, transmission, or spread of communicable diseases by HCT/Ps," not the broader safety/identity/strength/quality/purity mandate that governs drugs under Part 211. cGTP covers facilities, environmental control, equipment, supplies/reagents, recovery, processing, labeling, storage, and distribution (21 CFR §§ 1271.190–1271.265), generally using a "where appropriate" flexibility standard.

Critically, 21 CFR § 1271.150(d) states that if an HCT/P is also regulated as a drug (i.e., it fails the Section 361 four-criteria test and becomes a 351(a) biologic), the establishment must comply with both cGTP and full cGMP under Parts 210/211 — the two frameworks supplement, not replace, each other.

Aseptic processing and risk mapping

FDA's 2004 aseptic-processing guidance describes an ISO 5 "critical area" as the space where sterilized product, containers, and closures are directly exposed and must be protected — no more than 3,520 particles/m³ at ≥0.5μm, measured near the actual work site during filling/closing, with HEPA-filtered unidirectional airflow. FDA recommends the immediately adjacent area meet at least ISO 7 standards under dynamic (in-operation) conditions, with ISO 8 appropriate only for less critical activities like equipment cleaning. Positive pressure differentials of at least 10–15 Pascals are recommended between rooms of differing classification.

What FDA inspections check

Per FDA's Compliance Program Guidance Manual for licensed biological products, inspectors evaluate aseptic technique directly where possible, review environmental-monitoring procedures and excursion investigations, confirm personnel gowning-qualification programs (including microbiological surface sampling), and assess facility/HVAC design against both 21 CFR §§ 211.42–211.58 and 21 CFR § 600.11 side by side for biologics manufacturers.
